We appreciate your patience and understanding as we triage an influx of new patients who need care. We will contact you to schedule an appointment as soon as one becomes available. To help us serve you more efficiently, we kindly ask that you hold off on non-urgent calls. 

Please see below for the steps for becoming an Anchor Health patient.

How to become an Anchor Health patient. 1. Submit new patient interest form. 2. Complete new patient registration form. When an appointment becomes available, we'll email you a link to complete our registration form. Please check your spam or junk folder. A parent/medical guardian must sign for patients under 18 years old. 3. Paperwork is processed. Insurance is verified or Sliding Scale application is completed. We'll follow up if anything is missing. 4. Appointment is scheduled. We'll contact you within 5-7 business days to schedule your appointment and answer any questions you may have.
